Job Summary

The Case Manager will work to support the Mission of Mercy Housing and Shelter Corp. and the Haven Program. The YAS Program at Mercy Housing and Shelter oversees ten young adults ages 18-25. The Case Manager will work to ensure the highest quality of service within the mission of the organization and will ensure program residents are treated with respect and compassion. The Case Manager holds primary responsibility for working towards the goal of each client’s full and productive integration with the community.

Duties & Responsibilities

  • Knowledgeable of evidenced based practices both principles and techniques in all aspects of care (e.g., Integrated Dual Diagnosis, Person Centered Planning, etc.)
  • Able to provide individual, group and/or family supportive treatment under appropriate supervision.
  • Provide crisis intervention services.
  • The ability to participate in assigned nursing, clinical and/or rehabilitative interventions to meet client needs and identify support networks.
  • Document client contact, treatment, and progress.
  • Demonstrate the ability to advocate for services or resources that meet client needs and preferences.
  • Responsible for identifying client strengths and updates and modifies goals accordingly in the treatment plans.
  • Able to investigate alternative placement for clients.
  • Demonstrate the knowledge to address age, culture and gender specific characteristics when administering prescribed nursing interventions.

Requirements

A Master’s degree in Social Work or a related field is preferred, along with three to five years of experience in social work or behavioral science. Candidates must possess a valid driver’s license and demonstrate proficiency in computer applications and case management principles.

    Education and Experience

    • Master’s Degree in Social Work or related field preferred; plus, three to five years of experience in social work, rehabilitation therapy, or behavioral science.
    • Ability to work with multicultural clientele.
    • Clinical background with young adult experience preferred.
    • Knowledge of case management principles, practices, and procedures.
    • Considerable knowledge and experience in the dynamics of individual and group behavior.
    • Computer proficiency (i.e., databases, Microsoft Word, Excel), required.
    • Bilingual fluency (English/Spanish) preferred.
    • Certificates and Licenses
    • Valid driver’s license, clean driving record and proof of current automobile insurance, and reliable transportation required.

    Compensation & Benefits

    • $22-$24 per hour, based on experience.
    • Community Housing Advocates provides a generous employee benefits package, including medical, dental, retirement, short- and long-term disability, and tuition reimbursement. Full-time employees receive PTO days (paid time off) per year, plus 12 paid holidays.
