In this vital role within our Crisis Services Team, you will champion care coordination by facilitating resources, providing preventative services, and offering critical treatment and support. You will directly impact individuals navigating mental health or substance use challenges, including youth, older adults, and individuals with intellectual and developmental disabilities (IDD). WHAT IS THIS POSITION RESPONSIBLE FOR? 1. Conduct screening tools as a part of the assessment process that will be used by treatment team for further evaluation and review. 2. Provide phone, Zoom, and in person interviews with community members, or their families, to screen for mental health and substance use disorder needs and safety concerns. 3. Services are delivered utilizing a recovery/person centered philosophy and orientation, keeping with the preferences of the person receiving services and their care needs. 4. Confer with related health professions in relation to the need for interdisciplinary information sharing and warm hand off. 5. Provide Care Coordination through identification of needs and barriers and assist with accessing resources. Coordinate with other systems to meet the needs of those served, including criminal and juvenile justice and child welfare. 6. Monitor client's use of services, ongoing safety, and successful engagement with referrals. 7. Provide face-to-face services with individuals and families that utilize motivational interviewing and ongoing assessment of risk and needs; some services may be provided via phone or zoom. 8. Participate in relevant staff training and daily treatment team meetings. 9. Collaborate with the team to ensure the ongoing needs of people served are addressed. 10. Maintain appropriate records of services to reflect assessment of risk, safety planning, engagement, advanced psychiatric directives, and care coordination activities. 11. Coordinate access to continuum of care for individuals contacting Crisis Services. 12. Serve as Insurance Navigator for individuals seeking assistance with enrolling in an insurance program. 13. Assist the crisis services team with developing and accessing health interventions. 14. Assist those with limited English proficiency, or other communication challenges, in accessing services via appropriate methods. 15. Assist consumers with implementation of crisis prevention plans to prevent and deescalate future crisis situations. 16. Assist consumers with access to CCBHC services regardless of area of residence. 17. Act as liaison with local hospital inpatient units to assure continuity of care for Southwestern clients. 18. Support Crisis Services by answering Crisis Line calls and performing community responses as needed. 19. Serve as the point of contact for follow-up appointments after inpatient hospitalization for patients discharged to assure continuity of care for Southwestern clients. QualificationsWHAT'S REQUIRED FOR THIS POSITION? Bachelor's degree in social work, psychology, or other related degree with one year experience preferred. Must possess interest and ability to work with community members who are in crisis and have severe and persistent mental illness or substance use related issues. At least 1 year experience in care coordination or clinical practices. Experience with working with community partners and the unhoused population preferred; knowledge of local resources required. Ability to obtain and maintain certification as an Insurance Navigator. Obtain and maintain verbal intervention and de-escalation training. Requirements

A bachelor's degree in social work, psychology, or a related field is required, along with at least one year of experience in care coordination or clinical practice.
