Responsibilities
As a Physical Therapist, you will: Evaluate residents and develop individualized treatment plans Provide skilled physical therapy services in a skilled nursing and post-acute setting Improve patient mobility, strength, balance, endurance, and functional independence Monitor patient progress and modify treatment plans as appropriate Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and other healthcare professionals Educate patients and families regarding treatment goals, recovery expectations, and home exercise programs Complete accurate and timely clinical documentation in compliance with state and federal regulations Contribute to positive patient outcomes and quality-of-care initiatives
Qualifications
Current and unrestricted California Physical Therapist License required Graduate of an accredited Physical Therapy program Skilled Nursing Facility (SNF), rehabilitation, long-term care, or post-acute experience preferred Strong clinical assessment and treatment skills Excellent communication, organization, and time-management abilities Ability to work independently while contributing to a team-focused environment Commitment to delivering patient-centered care About Sonoma Post Acute At Sonoma Post Acute, we believe great patient outcomes begin with great clinicians.
