Structural Heart Nurse Coordinator Full Time I First Shift Baptist Health I Lexington, KY The Heart Valve Clinic provides specialized evaluation, coordination, and management of patients with valvular heart disease. The clinic collaborates with cardiology and cardiothoracic teams to deliver timely, evidence-based care, support treatment planning, and enhance patient outcomes through coordinated, patient-centered services.

Description

Structural Heart Nurse Coordinator is the point of contact for patients and families starting with the initial point of contact, telephone triage and the screening process. Position follows the patient through the continuum of care including inpatient admission, 30-day and one year follow-up. Ensures continuity of care through optimized patient assessment, clinical triage coordination, direct communication with physicians and daily patient management. Structural Heart Nurse Navigator will collaborate with the interventional cardiologists, cardiothoracic surgeons and clinical staff of multiple departments to coordinate the care of patients with structural heart disease, including those with aortic, mitral, and tricuspid valve disease who are being evaluated and/or selected for Transcatheter Aortic Valve Replacement (TAVR), Mitral Transcatheter Edge-to-Edge Repair (M-TEER), and Tricuspid Transcatheter Edge-to-Edge Repair (T-TEER), throughout their pre- and post-procedure care. Participates in the development of a marketing strategy, screening programs and other opportunities for program development. Ensures positive customer relationships by responding promptly to needs, managing difficult situations professionally, seeking feedback to improve service, providing timely assistance, and meeting commitments. Obtains STS risk score, Thoracic and Cardiovascular (TCV) in-hospital mortality score and performs frailty, functional and quality of life testing. Ensures accurate and thorough documentation to meet coding, STS registry requirements, certification, accreditation, and regulatory requirements (i.e. CMS requirements in National Coverage Decision for Transcatheter Aortic Valve Replacement (TAVR)). Initiates and drives performance improvement activities. Works closely with STS data analyst to ensure TVT registry data integrity and monitor patient processes and outcomes. Prepare case studies for morbidity and mortality reviews. Assists the professional development of staff or others when applicable to ensure the Structural Heart Disease educational content for orientation and annual education requirements remains current and applicable. Oversight of the Structural Heart Disease schedule and processes, collaborating with procedurals areas, IP areas, anesthesia, and appropriate personnel. Collaborates with members of the health care team to coordinate patient care.

Requirements

Bachelor’s degree in nursing from an accredited school. Minimum of three years of experience. Experience in Cardiology and/or Intensive Care required. Previous leadership and experience with projects. Basic Life Support certification via the American Heart Association within 90 days of hire.
