What you'll do

As a Case Manager, you will

  • Collaborate with multidisciplinary teams to provide holistic, person-centered services
  • Assist in the development, implementation, and review of recovery and treatment plans
  • Connect participants with community resources, benefits, and support services
  • Build and maintain relationships with community agencies and service providers
  • Deliver community-based services focused on recovery, education, and community integration
  • Transport participants to appointments and services as needed
  • Advocate for participants to access necessary medical, behavioral health, housing, and social supports
  • Complete assessments, service documentation, and reports in compliance with regulatory requirements
  • Monitor progress toward goals and support plan implementation
  • Maintain accurate records in accordance with organizational, funder, and regulatory standards
  • Identify service gaps and recommend solutions to improve participant outcomes
  • Perform additional duties as assigned

Qualifications

  • Required
  • High School Diploma or equivalent
  • Minimum two (2) years of experience supporting individuals in behavioral health, addiction recovery, or IDD services
  • Valid driver's license and reliable vehicle with required insurance
  • Intermediate computer proficiency, including Microsoft Word, Excel, and data entry systems
  • Ability to pass required background screenings, including FBI clearance if applicable
  • Strong communication, organizational, and problem-solving skills
  • Ability to collaborate effectively with participants, families, and community partners
  • Ability to multitask and remain focused in a dynamic environment
  • Ability to work effectively within a team environment
  • Familiarity with writing progress notes and service documentation
  • Preferred
  • Bachelor's Degree in Human Services or a related field
  • Minimum one (1) year of case management experience
  • Experience supporting individuals with intellectual and developmental disabilities (ID/DD)
  • Familiarity with creative arts-based programming
  • Knowledge of behavioral health, IDD services, and crisis intervention
  • Positive Behavioral Supports (PBS) knowledge and experience
  • Understanding of state and federal regulatory requirements
  • Certifications
  • CPR and First Aid Certification preferred (training provided upon hire)
  • PAC Certification preferred (training provided upon hire)
  • Medication Administration Certification preferred (training provided upon hire)
