VNA is looking for a Nurse Practitioner.
The Nurse Practitioner (NP) will provide clinical care, education, consultation, and support for research initiatives, while working collaboratively with physicians and other members of the multidisciplinary team. Responsibilities include coordinating patient care, overseeing individualized plans of care, conducting face-to-face home visits, completing clinician documentation, and providing follow-up care.
