The Habilitation Coordinator manages services for individuals with IDD in nursing facilities or community settings, including assessment, service planning, and advocacy. They are responsible for Medicaid billable activities, crisis prevention, and ensuring compliance with state and federal regulations.

Job Posting for Habilitation Coordinator-PASRR IDD

The Habilitation Coordinator manages services for participants in one of the following settings: nursing facility or community. In the nursing facility, the Habilitation Coordinator is responsible for PASRR duties. In community settings, the Habilitation Coordinator is responsible for performing enhanced community coordination (ECC) duties for individuals transitioning or diverting from institutional care. The Habilitation Coordinator is responsible for the following:

Assessment, service planning, monitoring, advocacy and crisis prevention and management for assigned caseload of individuals with IDD.

Provision of Medicaid billable Habilitation Coordination activities for individuals in a nursing facility.

Provision of Medicaid billable targeted case management activities for individuals in a nursing facility transitioning to the community.

On call response to PASRR Evaluation alerts within 72 hours.

Compliance with applicable local, state and federal rules and regulations.

Adherence to and promotion of the Texana Compliance Policy.

Travel within 6 county region.

Work hours: M-F, 8-5 with flexibility based on client need.

  • $24.50 to $27.25/hour based on years of experience
  • Required Education and/or Certifications, Skills
  • Bachelor’s degree in a social, behavioral, human services field, including but not limited to psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education and criminal justice.
  • Texas Driver License (in good standing)
  • Required Experience
  • At least one (1) year of experience working directly with individuals with ID or DD

  • Preferred Education and/or Certifications, Experience, Skills
  • Experience working with individuals with IDD who have mental health and behavior support needs or linking people with IDD to mental health supports
  • Experience in coordinating or providing services to individuals with IDD, including those with complex medical needs, in the community
  • Knowledge of State Rules relevant to PASRR, Habilitation Coordination, and Service Coordination
  • Strong organizational and computer skills (Microsoft Office and Electronic Health Record System)

Job related Physical Requirements

Must be able to perform SAMA and CPR. Training is provided to employees once hired

Must be able to perform moderate to heavy lifting (15#- 45#), kneeling, pushing, stooping, climbing stairs, walking, standing, and sitting

Must have ability to hear (with aid), see, write, operate office equipment, operate a motor vehicle and depth perception.

Qualified individuals must be able to perform these functions with or without reasonable accommodation.

Candidates must hold a bachelor's degree in a social, behavioral, or human services field and possess at least one year of experience working with individuals with ID or DD. A valid Texas Driver License is required, and the ability to perform physical tasks such as lifting and CPR is necessary.
