Position Summary

We are seeking a compassionate, skilled Registered Nurse (RN) to join our Home Care Team on a part-time basis during weekend day shifts. In this role, you will deliver high-quality, continuous nursing care and symptom management to patients living in their home environment, ensuring comfort and support for both patients and their loved ones.

KEY RESPONSIBILITIES • Direct Patient Care

• Conduct skilled nursing assessments, administer medications, manage symptoms, and perform hospice-specific interventions according to individualized care plans.

• Support: Provide end-of-life education, physical comfort care, and emotional guidance to patients and family caregivers.
• Interdisciplinary
• Collaboration: Work closely with physicians, medical directors, social workers, chaplains, and hospice aides to coordinate seamless, comprehensive care.
• Documentation: Maintain accurate, timely, and complete clinical documentation in compliance with Medicare/Medicaid regulations and organizational standards.
• Travel: Visit patients across the local service area within scheduled weekend hours.
