The Case Manager RN will: Perform Discharge Planning evaluations including risk assessments for defined population within expected time frame Provide Assessment and ongoing reassessment of a patient's diagnosis, prognosis, and anticipated care needs and goals.
Collaborate with other disciplines; actively participate in interdisciplinary rounds.
Collaboration includes progression of care, appropriate length of stay, integration of risk factors to reduce readmission, and integration for the care plan.
Collaborate with the social worker daily to identify patients in need of social work intervention or with potential post-hospital discharge needs.
Intervene and educate patients and families regarding anticipated complex home care needs including providers which are applicable to the patient’s goals and treatment preferences.
Utilizes teach back methodology.
Obtain patient choice for skilled home care providers and educates about payer in-network providers.
BSN preferred or must obtain within three (3) years of hire date Licensed as a Registered Nurse in the State of New Jersey Minimum of three (3) to five (5) years of experience in acute care nursing Good critical thinking and communication skills Must be willing to work on some weekend days and some holiday days
85,467.00 - 141,066.00 USD We offer competitive base rates that are determined by many factors, including job-related work experience, internal equity, and industry-specific market data. In addition to base salary, some positions may be eligible for clinical certification pay and shift differentials. The salary range listed for exempt positions reflects full-time compensation and will be prorated based on employment status.
