The Certified Nursing Assistant provides direct care and assistance with activities of daily living to participants in a clinic and community-based setting. They also participate in interdisciplinary care planning and maintain accurate documentation of services provided.
Under the supervision of the Clinic Manager, the CNA is responsible for providing and assisting participants with activities of daily living and other assigned duties within the facility.
Monitors health status and provides a safe environment for participants.
Functions as a member of interdisciplinary team.
Promotes professional working relationships with both internal and external customers.
Adheres to and supports all organizational policies and procedures and standards.
Essential duties and responsibilities
Assists participants with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) as assigned or needed.
Follows facility schedule for daily/weekly assignments.
Participates in participant’s care planning through cooperation with interdisciplinary teams.
May rotate into day center services as a CNA as applicable and deemed necessary by the Director of Clinical Services or Home Care Coordinator.
May rotate into the clinic as a CNA as deemed necessary by the Director of Clinical Services.
May rotate to provide Day Center escort for participants in the community as deemed necessary by the Director of Clinical Service.
Participates in Quality Improvement programs.
Handles potentially infectious specimens with appropriate biohazard precautions, and practices universal precautions.
Consistently cooperates and supports organization in problem solving issues.
Participates in continuing education classes and any required staff and training meetings.
Responsible for maintaining professional affiliations and any required certifications.
Other duties as requested/assigned
Assure compliance with infection control, personal protection equipment, blood-borne pathogens, hazardous material handling, and fire safety standards.
Attend staff meetings and participant care planning on a regular basis.
Maintain current written records, indicating ongoing documentation of services provided, reassessments of changing needs, and participant’s expressed wishes.
Prepare and submit timely written reports as required.
Candidates must hold a high school diploma or GED and be an active Certified Nurse Assistant in Maryland. At least one year of experience working with a frail or elderly population is preferred.
Maintain confidentiality of participant information.

Attend and participate in staff meetings, in-services, projects, and committees as assigned.
Adhere to and support the center’s policies, practices, and procedures.
Accept assigned duties in a cooperative manner and perform all other related duties as assigned.
Be flexible in schedule of hours worked.
May require use of personal vehicle.
