Job

Summary: Care Coordination Associate assist BCDSS permanency staff with coordinating health care services for children in out of home placement, including scheduling of appointments for routine medical, dental, mental health and specialty care as needed.

Duties and Responsibilities

  • Serve as a resource person for BCDSS permanency staff for navigating the medical, dental, and mental health systems.
  • Track health care services for children in out of home placement to assure their health needs are being met including retrieving medical documentation for medical case review
  • Identify health care resources and make referrals to appropriate agencies.
  • Teach and role model appropriate health-seeking behaviors to DSS staff, foster and kinship families
  • Participate in staff, community, professional and inter-agency meetings and conferences as necessary
  • Document assessments, activities, treatment plans, and resolutions in database.
  • Update medical information and MATCH database
  • Update medical information in CHESSIE
  • Answer the telephones and assist callers

Minimum Qualifications

  • Bachelor’s degree in social work or health related field
  • Experience in coordinating care/services for clients
  • Well developed interpersonal & communication skills
  • Proficient in MS Word, Excel, , Outlook and Gmail
  • Professional appearance and demeanor

Requirements

Candidates must hold a bachelor's degree in social work or a health-related field and possess experience in coordinating client services. Proficiency in Microsoft Office, strong communication skills, and the ability to manage multi-line phone systems are required.
