  • Function: The Medical Care Coordinator is a Licensed Practical Nurse (LPN) functioning under the administrative and medical supervision of a Registered Nurse (Supervising Nurse or their designee). The Medical Care Coordinator is responsible for supporting, coordinating, and promoting a full range of health services for students and clients participating in Waterford Country School programs, including but not limited to Education, Specialized Trauma Treatment, Assessment, and Reunification Home (STTAR), Intensive Transitional Treatment Center (ITTC), Therapeutic Boarding (TPB), Children’s Outpatient Mental Health Clinic (OPCC), and Camp Cuheca. They will be a member of the medical team, assisting the nurses and consulting physicians to maintain standards of best practice and incorporating the agency’s practice models, including but not limited to
  • CARE: Creating Conditions for Change model and Therapeutic Crisis Intervention (TCI). Hours are primarily Monday through Friday from 10 am to 6 pm. However, periodic availability on evenings and weekends may be required.

Duties and Responsibilities

Provide individualized health care education as needed for children participating in WCS programs. Weekly inventory of all medical supplies and medications on hand. Check expiration dates and replace as necessary. Ensure Medication Administration Reports (MARs) and individual health records have the most current, signed physician orders. Ensure all records have the most recent physical/immunizations, and routine medical appointment records. Prepare the medical discharge summary of all residents immediately prior to discharge. Compile travel medical folders for each resident, ensuring up to date information is included for both scheduled and emergency medical visits. Establish medical office charts upon notice of intake/admission. Complete routine/monthly checks of vital signs, height, weight. Document checks in medical records. Maintain security of all medications in WCS programs. Prepare written instructions and explanations to parents or proper person for administration of medication for children going home or on home visits. Maintain first aid kits and spill kits across the agency programs. Administer appropriate first aid to children and staff injured or becoming ill on duty. Coordinate and monitor care of children when they are ill or injured, assisting in treatments as directed by the Supervising Nurse. Maintain medical office equipment and supplies necessary for treatment and care of children and staff. Arrange for children’s medical appointments with physicians and caregivers, as directed by the Supervising Nurse. Use the CARE: Creating Conditions for Change model to guide interactions and philosophy of support for agency staff, volunteers and stakeholders. Inform the nurse(s) and/or consultant physician(s) on a timely basis of health care needs of children in our care, including but not limited to problems and emergency situations. Coordinate with pharmacies to ensure all youth medications are up to date, packaged when necessary, and MARs are correct with weekly counts and staff signatures. Maintain state and agency certifications: Including but limited to Administration Certification, CPR, TCI, CARE, Human Trafficking 101, Mandated Reporting, HIPAA and Confidentiality, and Suicide Awareness and Prevention. Administer medication to children in accordance with the State of Connecticut Medication Administration Certification standards and practices. Serve as a liaison and work collaboratively between the medical program and other departments within the agency. Maintain knowledge of and follow agency policies and procedures as contained in the WCS Personnel Handbook, WCS Procedures Manual, and department manuals. Function as a positive role model for all students and staff at the Waterford Country School. Maintain an awareness of and sensitivity to the cultural and socioeconomic differences present among the service population and fellow staff members of WCS, ensuring that cultural needs are respected and supported through the delivery of services and the development and implementation of agency programs and policies in a manner that is free from discrimination and bias relating to race, ethnicity, age, disability, gender, sexual orientation, status as a victim of domestic violence or religious beliefs. Maintain a valid Connecticut or home state driver’s license. Incumbent will maintain access to and comply with current and ethical professional standards of the discipline in which he/she is credentialed. Other duties as assigned. Limited Telework Capability Telework is limited to rare or emergency needs and requires advance approval from a Program Director. Any telework should be performed in accordance with the Remote Work Policy in the WCS Personnel Handbook. THIS JOB DESCRIPTION SHOULD NOT BE INTERPRETED AS ALL-INCLUSIVE.
