Provide specialized primary and palliative medical care to patients in person and via telephone. Maintain accurate patient records, participate in medical staff meetings, and assist in ensuring 24/7 medical coverage for PACE patients.

This position offers you the opportunity to provide specialized care for all people with complex needs related to life-changing illness. Key Responsibilities o Demonstrates the ability to administer quality primary care in a professional manner which supports the patient and families. o Provide documentation in the patient record that meets criteria for good medical practice and addresses legal mandates. o Provide on-call coverage for patient / families on a rotation basis (not applicable to all positions). o Communicate successfully with other Hope Healthcare services.

Knowledge & Skills o Adult Health, Primary Care and / or Geriatrics o Hospice or Palliative Care (preferable) o Practices within the standards of the American Nurses Association for: Scope and Standards of Practice, Code of Ethics for Nursing, Nursing’s Social Policy Statement and Specialty Nursing Standards

Required qualifications o Advanced Nursing Degree, Master’s level from accredited nursing school. Specialty in Adult Health, Primary Care, and Geriatrics preferred. o Current unrestricted APRN license, Florida. o Certification in Hospice and Palliative nursing preferred. o Must possess / be eligible for Medicare license (Medicaid preferred) as a participating provider. o DEA license required. o Must possess a valid driver’s license in the state of Florida, current automobile insurance, and reliable transportation. Certifications/Licenses/Registration: Must possess a valid driver’s license in the state of Florida, current automobile insurance, and reliable transportation. A current license to practice Advanced Nursing in the State of Florida is required. Credentialing through ANCC as a NP is preferred. Certification in Hospice and Palliative nursing is preferred. Active BLS for healthcare professionals from the American Heart Association or Red Cross. Certification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs. Experience Requirements Knowledge of the principles and practices of primary medical care and palliative medicine. Certification in Hospice and Palliative nursing is preferred. Position Specific Responsibilities (not all inclusive) Provide medical care for patients assigned in person, and by telephone communication with staff nurses.

Provide documentation in the patient record that meets criteria for good medical practice, addresses legal mandates, and adequately supports billing codes.

Communicate successfully with other Hope HealthCare Services employees, the public, and community physicians in a manner that represents Hope’s culture of a collaborative and positive work environment.

Assist the Medical Director in maintaining 24 hour, 7 day per week medical coverage for PACE patients in Hope Hospice facilities and services (if applicable).

Provide on-call coverage for Hope Palliative Care patients/families on a rotation basis (if applicable).

Participate in Hope PACE medical staff meetings and complete any mandatory continuing medical education activities as directed by the administration, in addition to CME needed to maintain certification.

Participate in any Quality Assurance activities approved by the administration as appropriate to your job setting.

Follow federal and state laws regarding medication prescription. Represent the company in a positive and professional manner at all times with others, e.g., co-workers, management, vendors, and customers.

Consistently represents Hope with integrity and professionalism.

Complete all job responsibilities in a thorough and complete manner, utilizing good judgment in all aspects of the job. Carry out duties and responsibilities in a manner that exemplifies excellent customer service.

When unsure of a directed task or any part of job responsibilities, proactively seek the assistance of immediate supervisor.

Assist with special projects, as requested.

All other job duties as may be assigned. Compensation Pay Range

$88,500.00 - $134,300.00 This position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as on-going compliance with the Drug-Free Workplace Policy. All Chapters Health System employees performing services for Florida affiliates are submitted through the Florida Care Provider Background Screening Clearinghouse to verify eligibility after a conditional offer of employment is made as well as ongoing eligibility. Requirements

Requires an Advanced Nursing Degree (Master's level) and a current unrestricted Florida APRN license. Candidates must also possess a valid driver's license, DEA license, and active BLS certification.
