Case Manager (Delta Landing)

Build rapport with clients and, when appropriate, support family engagement and reunification efforts.

Facilitate life skills development, including independent living, conflict resolution, and self-sufficiency.

Attend all required meetings, trainings, and program-related activities.

Prepare and submit reports as required by program leadership.

Perform other duties as assigned.

EDUCATION / EXPERIENCE • Education

• Associate’s degree (AA/AS) in Social Work, Public Health, Health Services Administration, or related field required.
• Bachelor’s Degree (BA/BS) in a related field preferred.
• Experience:
• Minimum of one (1) year of full-time case management experience in housing, homelessness, or behavioral health services; OR
• Three (3) years of full-time experience working with individuals experiencing homelessness in a social services setting (in lieu of degree), with a High School Diploma/GED.

• Certificates / Licenses / Clearances
• Valid California Driver’s License (preferred for field-based work and transportation of clients) clean driving record, and must be 21 years of age or older
• Successful completion of criminal background and fingerprint clearance.

Preferred Qualifications

• Bilingual and bicultural (Spanish preferred).
• Lived experience of homelessness and/or behavioral health system involvement.
• Experience in street outreach, shelter operations, or clinical case management.
• Familiarity with Contra Costa County Coordinated Entry and provider network.
• Experience with HMIS documentation, reporting, and compliance standards.
