Activities - BCN PACE Activities Assistant
The Activities Assistant plans, coordinates, and implements therapeutic, social, and recreational activities for PACE participants. They work closely with the interdisciplinary team to provide participant-centered programming that enhances quality of life and encourages independence.
As a Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely at home.
Our PACE experts work collaboratively with an interdisciplinary team to decide what’s best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services.
Most care services are provided at the PACE Center, including transportation to and from the Center.

The PACE Activities Assistant supports the planning, coordination, and implementation of therapeutic, social, and recreational activities for participants enrolled in the Program of All-Inclusive Care for the Elderly (PACE). This role promotes participants’ physical, cognitive, emotional, and social well-being through engaging designed to enhance quality of life, encourage independence, and support individualized care goals. The Activities Assistant works closely with the Interdisciplinary Team (IDT) to provide participant-centered programming in a safe, supportive, and inclusive environment. Position Qualifications: High school diploma or equivalent required. Experience working with older adults, individuals with dementia, or in a healthcare, adult day, recreational, or long-term care setting preferred. Previous experience in PACE program, adult day health center, or long-term care strongly preferred CPR/First Aid certification (or willingness to obtain) Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.
