- At Lighthouse, we strive to perform at a high level for our patients and their families. https://lighthousebehavioral.com/ Position Overview - Step-Down Navigator The Step-Down Navigator is responsible for meeting with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment.
- The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to Outpatient eligibility, into Midas for trending report purposes.
- He/ She will have a thorough understanding of the Step-Down Playbook, implementing components of the Playbook as needed.
- A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred.
- Responsibilities include, but not limited to: Meeting with eligible patients to discuss the benefits of step-down services as part of their continuum of care treatment.
- Communicating eligibility to senior leadership and associated departments.
- Coordinating between departments to ensure a smooth transition for patients.

Qualifications
Requirements
Education: High School graduation or GED required.
Experience
Minimum two (2) years’ experience in an inpatient psychiatric/mental health setting.
Licensure
Peer Support Certified, preferred. Current CPR, Physical and Verbal Crisis Intervention Certification, preferred. Training may be obtained during new hire orientation.
