Job Summary and Responsibilities As our Care Coordination Assistant, you will be a vital caregiver and the central hub, assuming responsibility for patient care processes from newborn to geriatric for a designated time frame.
You will directly contribute to safe, therapeutic patient care while serving as the primary receptionist and secretary.Every day you will support safe patient care through multidisciplinary processes and coordinating unit activities.
You will efficiently manage information flow and assist healthcare team members in delivering care across all patient age groups.To be successful in this role, you will demonstrate strong organizational and communication skills, a proactive approach to managing day-to-day operations, and a commitment to providing safe, therapeutic care while working effectively with the entire healthcare team.
Collaborates with and supports the Care Coordination team, including, but not limited to the RNCC and Social Worker.
Manage timely post-acute care referrals, to assist with length of stay management and mitigation of denials.
Communicate and collaborate with interdisciplinary teams, patients, and family to facilitate care progression.
Ensure post-acute follow-up appointments for identified patients to assist with readmission prevention and ensuring continuity of care.
Candidates must hold an Associate degree in a business or health-related field, or a high school diploma/GED. Preferred qualifications include at least 3 years of experience in healthcare, care coordination, or discharge planning, along with relevant clinical certifications such as LPN, CMA, or CNA.
Deliver required patient notifications and letters, and obtain patient or family signatures on required documentation as needed to meet regulatory compliance.
Manage transportation referrals as requested.
Job Requirements Required Associates Other Associate (two-year) Degree in Business/Health-Related field, upon hire or High School Graduate General Studies, upon hire or High School GED General Studies, upon hire and Preferred At least 3 years previous experience in health care, care coordination, utilization review, discharge planning or similar healthcare setting. and Bachelors Other or BSW, upon hire and Experience supporting clinicians in demanding and fast paced patient care environments.
Licensed Practical Nurse: WA, upon hire or Certified Clinical Medical Assistant, upon hire or Certified Medical Assistant AAMA, upon hire or Certified Medical Assistant NHA, upon hire or National Certified Medical Assistant NCCT, upon hire or National Registered/Certified Medical Assistant NAHP, upon hire or Registered Medical Assistant AMT, upon hire or Registered Medical Assistant, upon hire or Cert Med Asst - MMCI, upon hire or Certified Nursing Assistant: WA, upon hire or Emergency Medical Technician: WA, upon hire
