  • PURPOSE: The Certified Registered Nurse Practitioner (CRNP) serves as the advanced practice provider and clinical leader for Pittsburgh Mercy's Medical Respite and Mercy of Care programs. This role delivers scheduled, onsite, and transitional medical care to adults experiencing homelessness or housing instability, many of whom have serious mental illness, substance use disorders, and complex acute or chronic health conditions. Working collaboratively with program leadership and the Chief Medical Officer or designated physician, the Nurse Practitioner provides comprehensive clinical assessment and treatment, promotes medication safety, supports appropriate admissions, coordinates care across healthcare and community systems, and facilitates safe transitions from hospitals and emergency departments to community-based services. The position partners closely with the Pittsburgh Mercy Family Health Center, Operation Safety Net, behavioral health teams, hospitals, pharmacies, home health agencies, specialists, case managers, and community organizations to ensure integrated, person-centered care that improves health outcomes and supports recovery and stability.

  • WILL DO: Direct Clinical Care and Medication Safety for Mercy of Care and Medical Respite Programs Conduct comprehensive and focused medical evaluations, including admission histories and physical examinations, post-hospital assessments, and clinically indicated follow-up visits. Assess, diagnose, and manage acute and chronic medical conditions within the scope of licensure, clinical privileges, credentialing requirements, and available program resources. Provide or coordinate wound care, infection management, post-operative care, and ongoing monitoring following acute illness, injury, or hospitalization. Perform medication reconciliation at admission, following hospital or emergency department encounters, and upon discharge. Prescribe and monitor medications, identify potential interactions or adverse effects, address access and adherence barriers, and collaborate with pharmacies and prescribing providers to ensure safe medication management. Order, review, and follow up on laboratory tests and diagnostic studies. Coordinate specialty care and home health referrals and ensure clinically significant findings and recommendations are acted upon appropriately. Deliver preventive healthcare services, patient education, and evidence-based harm reduction and overdose prevention interventions. Provide treatment for substance use disorders, including medication-assisted treatment, when consistent with the clinician's qualifications, competencies, and approved privileges. Identify signs of medical or behavioral health deterioration, determine when emergency evaluation or a higher level of care is warranted, and initiate established escalation procedures. Maintain accurate, timely, and complete documentation of assessments, clinical decisions, treatment plans, orders, communications, and follow-up activities within the designated electronic health record and other approved systems.

Minimum Qualifications

Graduate of a Pennsylvania State Board of Nursing approved Nurse Practitioner program at the master's, post-master's, or doctoral level, or an equivalent program recognized by the Pennsylvania State Board of Nursing. Current Pennsylvania Certified Registered Nurse Practitioner (CRNP) certification and national certification in an appropriate specialty area, such as Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), consistent with credentialing and privileging requirements. Current Pennsylvania CRNP Prescriptive Authority Approval and active DEA registration, or the ability to obtain both prior to employment. Must maintain a compliant collaborative agreement as required by Pennsylvania regulations. Minimum of 3 years of clinical practice experience Preferred, including the care and management of medically complex adult populations. Current Basic Life Support (BLS) and Cardiopulmonary Resuscitation (CPR) certification. Ability to obtain and maintain all required Pittsburgh Mercy credentialing, payer enrollment, and clinical privileges necessary to perform assigned responsibilities. Experience in medical respite, primary care, hospital medicine, transitional or post-acute care, community health, or care of people experiencing homelessness; along with experience with serious mental illness, substance use disorders, harm reduction, overdose prevention, and medications for substance use disorders preferred. Experience with wound care, post-surgical recovery, medication reconciliation, and cross-system care transitions preferred. A valid driver's license is required. Requirements

Candidates must hold a master's, post-master's, or doctoral degree from an accredited Nurse Practitioner program and possess current Pennsylvania CRNP certification. A minimum of 3 years of clinical practice experience and active DEA registration are required.
