What you'll do

As a Case Manager, you will

Work as part of a multidisciplinary team to provide holistic, person-centered services.

Assist individuals in developing and achieving recovery goals.

Coordinate services and connect participants with community resources and supports.

Build strong relationships with community agencies and service providers.

Advocate for participants to ensure their needs are met and barriers are addressed.

Provide community-based case management services throughout the service area.

Transport participants to appointments and community resources when needed.

Complete assessments, service plans, reviews, and required documentation.

Maintain accurate and timely records in accordance with agency and funder requirements.

Collaborate with clinical staff, medical providers, and community partners to support positive outcomes.

Qualifications

  • Required
  • Two (2) years of experience providing support to individuals in behavioral health, addiction recovery, unhoused services, or IDD/BHH programs
  • Intermediate computer skills
  • Strong communication and organizational abilities
  • Ability to work independently and in community-based settings
  • Preferred
  • Bachelor's Degree in Human Services or a related field
  • One (1) year of case management experience
  • Knowledge of behavioral health resources and community support systems
  • Schedule
  • Full-Time
  • Primarily community-based work throughout Montgomery County
  • Flexible schedule based on participant and program needs
  • Some evening or weekend availability may be required
  • Physical requirements
  • Ability to lift up to 50 pounds
  • Ability to stand, sit, walk, bend, reach, and climb stairs as needed
  • Ability to travel throughout the community and transport participants as required
  • Ability to perform essential job functions with or without reasonable accommodation
