SHARE (Supportive Housing Assistance to Reach Excellence) is a supporting housing service designed to assist persons with serious mental illness(es) in maintaining independent housing in the community. The case managers provide the support needed in each person's home environment and in the community. The case manager assists with skill development, adherence to and progress towards treatment plan goals, and coordination of care.

Contribute to the development, implementation, monitoring, and updating of Individual Recovery Plans (IRPs). Provide and document therapeutic rehabilitation and skill-development services that promote independence and community functioning. Maintain accurate, timely, and confidential clinical and medical records. Monitor clients’ ongoing health needs, particularly those at risk due to chronic or acute conditions, and make appropriate referrals. Educate clients and families on nutrition, medication use, symptom management, food-drug interactions, and discharge planning. Facilitate therapeutic, educational, recreational, and social activities that promote engagement and recovery. Provide crisis intervention, including assessment, de-escalation, safety planning, and coordination with crisis or hospital services. Coordinate and advocate for appropriate medical, behavioral health, and community services based on client needs. Support clients with daily living, transportation, community integration, and independent living skills. Promote wellness and recovery by helping clients develop coping strategies, recovery goals, and individualized wellness plans.
