RN Case Manager Job Responsibilities
- Completes discipline-specific initial and ongoing assessments. Conducts home visits and home assessments as needed, and consults w/ families and other supports for additional information as needed. Develops and maintains care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports. Follow up on all services to ensure quality/coordination and effectiveness of services.
