About the role

The RN Case Manager will conduct home visits and assessments to develop and maintain comprehensive care plans for elderly individuals and adults with disabilities. They are responsible for coordinating services and monitoring the effectiveness of care to ensure members live independently in the community.

Community Care is hiring a RN Case Manager to join our Family Care Program team. This rewarding opportunity is perfect for someone who enjoys team collaboration, community case management, being part of a mission driven organization and who shares our mission to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This is a full-time opportunity working out of our Waukesha location. *****$10,000 Sign On Bonus (External Hires)******

RN Case Manager Job Responsibilities

  • Completes discipline-specific initial and ongoing assessments. Conducts home visits and home assessments as needed, and consults w/ families and other supports for additional information as needed. Develops and maintains care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports. Follow up on all services to ensure quality/coordination and effectiveness of services.

Requirements

Candidates must hold an Associate degree in Nursing and a valid Wisconsin Registered Nurse license. A minimum of two years of recent experience working with older adults or individuals with disabilities is required.
