  • HourlyApplications must be submitted by 11:59 p.m. on September 15th, 2026. Under direction, performs work in providing social work services on cases involving complex social problems and social treatment plans; performs responsible and difficult casework, directs service programs, or provides work direction to paraprofessional staff; and performs related work as assigned. SNBC Case Management Activities Responsible for connecting with new contacts and referrals according to health plan guidelines. Provides appropriate referrals for additional assessment or services to meet the member’s needs. Maintains communication with member, family members, county staff, community providers to ensure access to services and coordination of care. Provides annual in person contact per health plan guidelines and ongoing care coordination as appropriate. Maintains appropriate documentation in SSIS and Appxtender. Completes a HRA assessment and Support Plan annually according to health plan guidelines and requirements. Outreach and Advocacy Activities Performs outreach contacts to current members, and to eligible consumers who are currently not receiving case management, to provide member education and promote the use of preventative services to improve member health status related to condition management. Provides advocacy services to assist the member to navigate the health care system, social service system and financial system. Coordinates with Income Maintenance Department when issues occur regarding member terminating due to lack of timely filing of paperwork for continuing eligibility or change of program eligibility. SNBC Support Provides support at the local level through education of family members, county staff and providers regarding programs and services available through SNBC. Assists with Quality Improvement Projects and Performance Improvement Projects including outreach activities, data collection and monitoring. Provides basic CCM support and training to other county staff when needed. Participates in SNBC scheduled unit meetings and trainings. Conducts care coordination duties according to the Person-Centered Protocol. Conduct MN Choices Assessments as assigned Contacts client, assess need for MnChoices assessment for initial assessments as assigned. Completes MnChoices assessments within required timeline. After initial MnChoices assessment complete CSP within required timeline. required assessment paperwork. Additional

Intake, crisis intervention to facilitate early treatment and rehabilitation and education for various target populations and the general public. To be available on a rotating basis to cover intake services and assure assigned days are covered. Attends training/workshops outside agency as applicable/available. Position RequirementsKnowledge, Skills, and Abilities Knowledge of the principles and techniques of interviewing and recording in social work and the ability to apply them. Knowledge of individual and group behavior. Knowledge of social casework objectives, principles, and methods. Knowledge of socio-economic factors, which affect the individual in their community. Knowledge of the principles of community organization. Knowledge of physical and mental illnesses. Knowledge of social welfare research methods. Knowledge of family systems and dynamics. Knowledge of characterizes/symptoms of children’s mental health diagnosis. Ability to effectively apply casework knowledge and skills. Working ability to work constructively within an agency in the community setting and in effectively utilizing appropriate resources and services. Ability to work constructively in the development and coordination of community resources to meet special needs. Ability to manage time productively. Ability to prioritize caseloads. Ability to communicate effectively, both orally and in writing. Ability to work under limited supervision with general autonomy in determining how objectives are achieved; supervisors generally set operating benchmarks, goals, and objectives. Ability to apply procedures, policies and/or precedents requiring analytical ability in adapting standard methods to fit facts and conditions using generally prescribed procedures. Ability to maintain effective communication and working relationships with numerous outside and inside contacts to achieve work objectives. Education and Experience Bachelor’s degree from accredited university/college with a major in Social Work or other approved social services degree. Valid MN driver’s license required. Noise levels within the workplace are described as moderate.
