Case managers provide comprehensive care coordination and health promotion for justice-involved individuals with behavioral health needs. They collaborate with a multidisciplinary team to develop person-centered goals and assist participants in accessing community resources and entitlements.

Job Title: Case Manager

Job Summary

CASES is a New York City agency at the forefront of the intersection between behavioral health and involvement in the criminal justice system. Our mission is to increase public safety through innovative services that reduce crime and incarceration, improve behavioral health, promote recovery and rehabilitation, and create community success opportunities. The Behavioral Health division consists of a continuum of services for justice-involved individuals with serious mental illness. The Nathaniel Clinic located in the Bronx provides outpatient mental health and substance use services to adolescents and adults, focusing on providing care to those involved in the criminal legal system.

Case Managers provide core services, including comprehensive care management, care coordination, and health promotion. Encourage and assist eligible clients to enroll with insurance plan of their choice and assist with housing, medical, dental and other community resources. Case Managers are part of a multidisciplinary team working collaboratively with therapists, psychiatrists/PNPs, and nurses to assist with person-centered goal planning, collaborate with participants, and support systems to identify immediate needs and service requirements and recommend treatment objectives.

Shift Hours

  • This position is full-time Monday –
  • Friday from 9:00am to 5:00pm.

Salary

The salary range for this role is $55,000 - $58,000 per year.

Location Address

424 East 147th Street, Bronx, NY 10455. Our office is easily accessible by public transportation.

Workplace Flexibility

Flexible - Staff have the flexibility to complete their workday from home or another suitable space, typically after reporting to the office and/or engaging in field or community-based work.

What You Will Be Doing

  • Assist with developing person-centered goal planning
  • Screen and assess individuals’ social drivers of health and assist with housing instability, food, transportation needs and other concerns impacting wellness
  • Collaborate with participant and care management team to identify services based on individual needs and recommend treatment objectives
  • Support program participants providing care coordination, care management, and health promotion
  • Assist eligible clients to enroll in health insurance/Medicaid, and identify other entitlements
  • Provide participant and family support and referrals to community and social support as needed
  • Participate in case conferences with ATI program staff and team members as needed
  • Assist participants with linkages and navigating systems within the behavioral health, human services, and criminal justice systems as appropriate
  • Use motivational interviewing, recovery, and trauma-informed approaches when delivering services
  • Provide individual community support, assist with entitlements, one-shot deals, and other community resources
  • Connect with family, significant others and support system, as identified by the participant, to promote recovery goals
  • Contribute to ongoing organizational commitments to diversity, equity, inclusion, and antiracist practices in delivery of services and work
  • Provide culturally competent services in accordance with CASES policies and practice
  • Any other duties as required by senior clinical leadership

What We Are Looking For

  • Bachelor’s degree in human or social services or related field
  • At least 2 years of work experience providing peer support to individuals with serious mental illness and/or substance use disorder and criminal justice involvement
  • Individuals with lived experience with mental health, substance use or criminal legal experience are encouraged to apply
  • Proficiency with computer and electronic and health records

Additional, preferred skills we are looking for but are not required

• Bilingual in Spanish / English

Candidates must possess a bachelor's degree in human or social services and at least two years of experience providing peer support to individuals with mental health or substance use disorders. Proficiency with computer systems and electronic health records is required.
