Job Summary Will assist in providing comprehensive case management services to the eligible population who have problems with substance use/abuse or dependence, as well as provide support for their families and significant others. Will also make necessary referrals and coordinate services, by promoting linkages between substance abuse treatment programs, health clinics and other social service providers to ensure that clients have access to all available health and social services necessary to obtain an optimum level of functioning. Make frequent contact with clients in their homes and natural settings to focus on practical problems of daily living, promote assertive advocacy, assistance with transportation if needed. Required Education and Work Experience Bachelors degree in Sociology, Psychology, Social Work, or Human Services. ~AND~ Two (2) years of related experience. Essential Functions Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Conduct extensive, culturally sensitive outreach in the community to identify individuals and families who would benefit from program services as well as conduct comprehensive assessments intake and orientation on program participants in order to determine individual and family needs. Collect necessary data as needed to assist with programmatic evaluation. Maintain accurate, neat, timely, and appropriate documentation of services provided on client file. . Provide support activities to motivate clients on the caseload to change self- defeating behaviors through the use of Motivational Interviewing and the Stages of Change Model. Support shall include follow- up and support for discharge planning during and following intervention services. Engage clients and encourage retention in program services to increase completion rates; this may include crisis interventions, encouraging family involvement and providing education. Make frequent contact with clients through home visits, telephone calls and office visits in order to encourage and offer support for a positive lifestyle. Identify family risk factors and help improve the health and functioning of the family unit by engaging the family members in support and involvement in the intervention/prevention process. Meet with clients and their families on a regular basis to monitor and follow up on their needs and preferences Provide case management services in settings that are based on the needs of the clients and the goals of the client's service plan, including office-based, home-based or community based locations. Promote and encourage entry into substance abuse and/or mental health services and make referrals, if appropriate, for clients who are in need of formal treatment. Provide case management activities that enhance the motivations of clients to reduce their risks of transmitting HIV and STD'S due to substance abuse and sexual behavior, and provide appropriate harm and risk reduction information and referrals for clients and significant other if necessary. Conduct HCV Testing to include both risk assessment and risk- reduction counseling ensuring that HIV testing is performed through the use of blood samples (such as finger stick-based testing). Ensure that participants testing HIV, HBV, and HCV positive shall be linked to the appropriate organization to obtain a confirmatory test and to the suitable medical provider. Promote and advocate for coordinated HIV, HBV, AND HCV case management and medical care efforts and collaborate with medical care providers to ensure clients obtain the appropriate medical care. Physical Demands The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to walk, talk or hear. This position requires the ability to move throughout the building and occasionally lift office products and supplies, up to 20 pounds. Additional Eligibility Qualifications Have knowledge of the agency's programs in order to refer individuals who seek information. Maintain working knowledge of personnel and office policy and procedures. Ability to express self, in English, in a clear, concise, understandable manner, both orally and in writing. Bilingual (English/Spanish) preferred. Reflect a pleasant attitude that represents Aliviane, Inc in a positive manner within the community at large, and maintain a professional attitude to the employees, clients and public at all times. Project the same positive manner while attending community activities Ability to gain and uphold the confidence and trust of vulnerable populations served by Aliviane, Inc. Strives for personal and organizational excellence. Ability to work in a collaborative manner. 9. Able to accept responsibility; follow or exceed organizational standards. Requirements

Candidates must hold a bachelor's degree in Sociology, Psychology, Social Work, or Human Services. Additionally, at least two years of related experience is required for this position.
