The Program Coordinator is responsible for delivering and implementing behavioral health or recovery programs within the facility. They provide direct patient care, assist with daily needs, and collaborate with the treatment team to achieve care goals.
Responsible for the delivery and implementation of behavioral health or recovery programs within a facility.
· Provide patient care to include coordinating activities, providing companionship and assisting with basic daily needs (dressing, bathing, eating). · Collaborate with treatment team members to assist with patient treatment and/or care goals. · May serve as a co-lead for groups. · Monitor the daily well-being of patients and identify need for social intervention. · Ensure all documentation regarding the program, such as care, treatment or incidents, is completed in a clear, concise, professional and completed in a timely manner. · Demonstrate a positive, empathetic and professional attitude towards customers always. When patient needs are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority.
