We're looking for a PRN Case Manager RN to join our Case Management team! We will hire a Registered Nurse or MSW for hospital discharge planners onsite at Valley View Hospital. Case Management and Epic experience a plus!
Previous Case Management experience strongly preferred!
1. Assists Valley View inpatients and outpatients and their families in the overall Management of their plan of care across the continuum.
BSN RN preferred.
Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program
State licensure as a Registered Nurse (RN).
1 year of nursing (RN)
$39.96 to $59.95/hour +14% PRN Differential Pay.
Entry salary dependent upon education, skill set, and experience.
Valley View recognizes a 4% pay difference for RN-ADN, BSN, MSN
