RN Case Manager

• Community Care is hiring a RN Care Manager to join our Family Care Program team. This is a full-time opportunity and this position will serve for our Ozaukee County *****$8,000.00 Sign on Bonus****
• RN Care Manager Job Responsibilities: Completes discipline-specific initial and ongoing assessments. Conducts home visits and home assessments as needed, and consults w/ families and other supports for additional information as needed. Develops and maintains care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports. Follow up on all services to ensure quality/coordination and effectiveness of services. RN Care Manager Job Requirements: Education: Associates degree in Nursing Required, Bachelors Degree preferred.
