Job summary
Work with community organizations, government agencies and property owners to secure living arrangements, negotiate rents, deposits, and move-in terms on behalf of clients. Manage lease agreements for short-term, transitional, and permanent housing.
Responsibilities
- Reasonable accommodation may be provided for individuals with disabilities to perform essential job functions.
- Conduct weekly housing inspections, follow reporting requirements, and work with clients to remediate issues.
- Provide case consultation, crisis intervention, discharge placement, and targeted case management services to assist clients in meeting basic needs and housing needs.
- Complete accurate and timely documentation as contractually required and meet minimum requirements for direct billable client services.
- Locate housing opportunities to meet short-term, transitional, and permanent housing needs and manage lease agreements.
- Prepare subsidy applications, maintain client housing files, and ensure rent calculations are accurate.
- Assist in monthly tenant rent collection, transport clients to housing locations and help them secure required documents for housing placement.
- Conduct housing screenings and assessments utilizing the coordinated entry system required.
- Shift coverage including part of the on-call team.
- Provide field-based services as required by program and client need.
- Maintain confidentiality of client’s care.
- Participate in scheduled meetings.
- Other relevant duties as assigned.
Qualifications
- Bachelor’s degree and 1 year experience or associate degree and 3 years' experience or 5 years' relevant experience. Verify this is required.
- High School Diploma or GED required.
- Valid CA driver’s license and current auto insurance per contract requirements required.
- Knowledge of housing needs of individuals with behavioral health needs preferred.
- Cultural competency understanding required.
- Bilingual preferred.
- Knowledge of Microsoft Office and EHR software preferred.
