The Role Under general direction, the Nurse Practitioner is responsible under direction of a physician for providing general medical care, preventative care and treatment to patients in one of the agency’s clinics.
This includes diagnosis, treatment, consultation and follow-up care.
This individual attends to the patient’s need with the utmost patient care, compassion and understanding.
This work is carried out in support of the mission and goals of Samaritan Daytop Village.
What You Will Do Performs routine physical examinations and preventive health measures within prescribed guidelines and instructions of a physician based on assigned program. Orders, interprets and evaluates diagnostic tests to identify and assess patient’s clinical problems and health care needs. Records accurate medical histories, physical findings and formulates plan and prognosis based on patient’s condition. Documentation includes medical evaluations, diagnosis, procedures, treatment, prognosis, referrals, education and consultation. Discusses case with physician and other health professionals and therapeutic team to prepare comprehensive patient care plan. Submits health care plan and goals of individual patients for periodic review and evaluation by physician. Prescribes medications and monitor effectiveness, or other forms of treatment such as physical therapy, inhalation therapy or related therapeutic procedures. Refers patients to physician or to specialized health resources for treatment. Makes patients aware of importance of a healthy lifestyle. Educates patients and their families on their preventive care, medical issues and use of prescribed medications and treatments.
Who You Will Be NYS RN and NP (Adult and Family specialty areas) licensures. Valid DEA registration. eMedNY Provider. Valid NPI. BLS Certification. Masters of Science in Nursing Degree from an accredited Advanced Nurse Practitioner program either in Family or Adult. At least One-Two (1-2) years experience working as a Nurse Practitioner preferably in addiction medicine or substance use and/or mental health treatment programs. Knowledgeable about federal and state regulations governing medical/clinical treatment in substance use treatment programs Ability to demonstrate sensitivity to patients with a variety of psycho social issues. Ability to work with a medical and clinical interdisciplinary staff to ensure appropriate management of clients. Computer literacy including proficiency in Microsoft Office Suite and EMR. Excellent interpersonal, communication, analytical, presentation and organizational skills. Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
