About the role

The Licensed Practical Nurse will coordinate resident care, administer medications, and monitor changes in resident conditions to ensure health and well-being. They will also provide direct nursing support, document services accurately, and supervise resident-care staff within the scope of practice.

In this role, you will help coordinate resident care, administer medications, monitor changes in resident condition, communicate with families and providers, and support resident-care team members. You will work closely with the Director of Nursing or Resident Care Director to promote residents’ health, safety, dignity, independence, and overall well-being.

This is a great opportunity for an LPN who enjoys building relationships with older adults and providing hands-on nursing support in a resident-centered environment.

  • What You'll Do
  • Administer medications and treatments in accordance with provider orders, community policies, and applicable nursing standards
  • Monitor residents for changes in physical condition, behavior, cognition, mood, appetite, mobility, or daily functioning
  • Promptly report significant changes and take appropriate action within the LPN scope of practice
  • Provide direct nursing care and resident support as needed
  • Assist with resident assessments and service-plan reviews under the direction of the appropriate nursing leader
  • Help implement and maintain individualized resident service plans
  • Document medications, treatments, observations, incidents, and nursing services accurately and timely
  • Communicate with residents, families, providers, pharmacies, and community team members
  • Receive, review, and carry out provider orders within the LPN scope of practice
  • Coordinate pharmacy needs, medication refills, and medication-delivery concerns
  • Perform medication-cart, treatment-record, and documentation audits as assigned
  • Support admissions, transitions, hospital returns, and changes in resident condition
  • Provide resident and family education within the nurse’s training and scope of practice
  • Promote wellness, safety, infection prevention, hydration, nutrition, and fall-prevention practices
  • Respond to resident emergencies and initiate appropriate interventions
  • Notify the appropriate nursing leader, provider, family member, or emergency service when required
  • Participate in resident-care meetings and communicate relevant changes to the Wellness team
  • Provide guidance and supervision to resident-care staff
  • Delegate nursing tasks only when permitted by New Hampshire law, the nurse’s scope of practice, community policy, staff training and competency, and the resident’s condition
  • Monitor the completion and effectiveness of delegated tasks
  • Assist with staff education, competency reviews, and required in-service training
  • Maintain resident privacy, dignity, rights, and confidentiality
  • Follow infection-control, medication-management, safety, and emergency procedures
  • Complete additional nursing and Wellness duties as assigned within the LPN scope of practice
  • What We're Looking For
  • Current and valid license to practice as an LPN in New Hampshire or a valid multistate nursing license with authorization to practice in New Hampshire
  • License must be active and in good standing
  • Knowledge of practical nursing standards and medication-administration practices
  • Strong resident-assessment, observation, and clinical judgment skills within the LPN scope of practice
  • Ability to recognize and report changes in resident condition
  • Strong documentation, organization, and time-management skills
  • Ability to communicate clearly and effectively in English, verbally and in writing, as required to understand provider orders, document nursing care, communicate changes in condition, administer medications safely, and respond during emergencies
  • Ability to communicate professionally and compassionately with residents, families, providers, and coworkers
  • Ability to provide direction, supervision, and appropriate delegation to resident-care staff
  • Ability to work independently while knowing when to consult or escalate to the appropriate nursing leader or provider
  • Experience working with older adults is preferred
  • Previous assisted living, memory care, long-term care, home care, or similar nursing experience is preferred
  • Dependable attendance and punctuality
  • Ability to stand, walk, bend, reach, and remain active throughout the workday
  • Ability to safely lift, transfer, push, or pull up to (physical requirement) pounds with assistance or available equipment
  • Ability to successfully complete required background checks, health screenings, and employment requirements
  • New Hampshire Nursing Requirements
  • The employee must maintain an active nursing license or valid privilege to practice in New Hampshire throughout employment
  • Nursing duties must be performed within the New Hampshire LPN scope of practice
  • Delegation decisions must consider the resident’s condition, the nature of the task, the employee’s training and demonstrated competency, required supervision, and applicable law and community policy
  • The LPN remains responsible for appropriate supervision and evaluation of delegated nursing tasks
  • Duties requiring an RN, APRN, physician, or other licensed provider must be referred or escalated appropriately
  • Medication administration and nursing documentation must follow applicable New Hampshire requirements and community procedures
