Position Summary

The Admission Nurse serves as the first point of clinical contact for residents entering Providence Pointe and plays an important role in creating a safe, organized, and welcoming transition into our care. This position completes initial clinical assessments, reviews medical histories and medications, coordinates admission information with the interdisciplinary team, and ensures required clinical documentation is complete and accurate. The ideal candidate combines strong clinical judgment with exceptional organization, communication, and attention to detail.

Key Responsibilities Complete comprehensive nursing assessments for newly admitted residents, including vital signs and immediate clinical needs.

Review and document medications, allergies, diagnoses, medical history, previous procedures, and advance directives.

Review admission orders and ensure necessary clinical information is communicated to the appropriate members of the care team.

Coordinate with physicians, nursing leadership, case managers, floor nurses, therapy, and other disciplines to support a smooth transition of care.

Complete required admission documentation accurately and within established timeframes.

Identify and escalate changes in condition or immediate clinical concerns.

Orient residents and families to the facility and help answer questions regarding the transition into care.

Support development and implementation of the resident's initial plan of care.

Maintain compliance with facility policies and applicable state and federal requirements.

Promote a welcoming, compassionate admission experience for residents and families.

Requirements

Qualifications & Requirements Active, unencumbered Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Kentucky. Graduate of an accredited nursing program. Current BLS/CPR certification. Clinical nursing experience required; long-term care, skilled nursing, rehabilitation, med-surg, or acute-care experience preferred. Experience completing admissions or comprehensive nursing assessments preferred. Strong clinical assessment and critical-thinking skills. Strong organization and attention to detail, with the ability to manage multiple admissions and priorities. Excellent written and verbal communication skills. Experience with electronic health records; PointClickCare (PCC) experience preferred. Ability to communicate effectively and compassionately with residents, families, physicians, and interdisciplinary team members.
