Seeking a compassionate, detail-oriented, and patient-focused Primary Care Nurse Practitioner to provide comprehensive healthcare services to patients across the lifespan. The NP will work collaboratively with physicians, nurses, and interdisciplinary staff to deliver high-quality primary and preventive care in an outpatient setting. The ideal candidate is committed to improving patient outcomes, promoting wellness, and providing culturally sensitive care within the community.
Conduct comprehensive patient assessments, including medical histories, physical examinations, and diagnostic evaluations Diagnose and treat acute and chronic medical conditions Develop and implement individualized treatment and care plans Prescribe medications and therapies in accordance with state regulations and practice guidelines Order, interpret, and review laboratory tests, imaging, and other diagnostic studies Provide preventive care services, including annual physicals, screenings, vaccinations, and health education Manage chronic diseases such as diabetes, hypertension, asthma, and cardiovascular conditions Collaborate with physicians, specialists, and healthcare team members regarding patient care plans Maintain accurate and timely electronic medical records (EMR) documentation Educate patients and families on disease prevention, medication management, and healthy lifestyle choices Participate in quality improvement initiatives and compliance activities Ensure adherence to HIPAA regulations and organizational policies
Current New York State Nurse Practitioner license Board Certification DEA registration Experience with EMR systems preferred Strong clinical judgment, communication, and interpersonal skills Schedule & Work Environment Full-time Outpatient primary care setting
