The CNA will provide personal care and support to hospice patients, including lifting, transferring, and assisting with daily hygiene. They are also responsible for reporting patient condition changes to the RN and maintaining accurate service records.
Follows written instructions for patient care as prepared by an RN or Clinical Director in the hospice plan of care. Assists patients with personal care (which includes lifting and transferring patients without assistance) to preserve good personal hygiene while maintaining a healthful, safe environment. Performs simple procedures as an extension of therapy services, such as ambulation, ROM and exercise. Reports changes in patient’s condition and needs to supervisor or RN. Maintains record of services performed and of apparent condition of patient. Establishes and maintains a good working environment with the patient/family and co-workers of the Hospice. Participates in patient care conferences to provide input requiring continuum of patient care. Perform household services essential to health care at home.
