As a QMAP, you are often the hands and eyes closest to our residents each day, and every medication pass, care task, and check-in is a chance to turn necessary care into a moment of real human connection. This role does far more than administer medications; it safeguards resident health while shaping how residents experience their home.

Summary

The QMAP provides safe medication administration, direct personal care, and thoughtful resident support that promotes dignity, independence, comfort, and quality of life. This individual assists residents with activities of daily living, responds to resident needs and call systems, observes and reports changes in condition, documents care accurately, and helps create a safe, welcoming home for every resident. This role requires sound judgment and disciplined prioritization, consistent with Colorado Department of Public Health and Environment (CDPHE) assisted living regulations.

Essential

Duties & Responsibilities

  • The following duties are representative of the position. Other duties may be assigned.
  • Administers medications safely, accurately, and on time in accordance with Colorado QMAP requirements, physician orders, resident-specific instructions, and training.
  • Reviews the medication administration record (MAR) prior to administration and documents immediately and accurately afterward.
  • Secures, stores, counts, and disposes of medications per policy; promptly reports discrepancies, refusals, and medication errors.
  • Observes residents before, during, and after medication administration and reports concerns or changes in condition to the licensed nurse or supervisor without delay.
  • Provides hands-on assistance with bathing, dressing, grooming, oral care, toileting, continence care, and other personal-care needs identified in the care plan.
  • Assists with transfers, ambulation, repositioning, and mobility or adaptive equipment according to training and the resident's plan of care.
  • Supports meals and hydration, including escorting residents, setting up meals, and providing permitted feeding assistance.

Requirements

Candidates must hold a current Colorado QMAP certification in good standing. A high school diploma or GED is preferred, along with the ability to perform physical tasks such as lifting and assisting with resident mobility.

    Education & Experience

    High school diploma or GED preferred. Current Colorado QMAP qualification/certification in good standing required, with ability to remain current throughout employment. Experience supporting seniors in assisted living, memory care, long-term care, or home care preferred.

    Language Ability

    Ability to read and interpret care plans, medication orders, and safety procedures; ability to document accurately and communicate clearly and respectfully with residents, families, and coworkers.

    Math Ability

    Ability to accurately calculate medication dosages, measurements, and related figures.

    Reasoning Ability

    Ability to apply sound judgment and disciplined prioritization to direct-care needs, including recognizing and promptly escalating changes in resident condition.

    Computer & Systems Skills

    Basic computer proficiency for accurate, timely documentation in medication administration and electronic care-record systems.
