We are seeking a Licensed Physical Therapist Assistant (LPTA) to join our Therapy Department at Jamestowne, a short-term rehab facility with an additional focus on outpatient services, with Community First Solutions.
In this role, you’ll work under the supervision of a licensed Physical Therapist to provide high-quality therapy treatments that support rehabilitation goals and help residents restore mobility, strength, and independence.

You hold a current Ohio license (or are license-eligible) as a Physical Therapist Assistant.
You bring at least one year of experience in geriatrics, rehabilitation, or long-term care (preferred).
You are skilled at motivating residents and supporting them in achieving their therapy goals.
You have strong communication skills, a collaborative mindset, and a passion for person-centered care.
You are familiar with therapy best practices, resident safety, and electronic health record documentation.
