Columbus, OH 43215Are you passionate about helping individuals access behavioral health care during critical moments? As a RREACT High-Touch Navigator, you'll play a key role in connecting individuals experiencing mental health and substance use challenges with the services they need following an emergency department (ED) visit. Working collaboratively with emergency department staff throughout Franklin County, you'll provide compassionate, person-centered support to ensure individuals successfully transition from ED acute care to ongoing outpatient behavioral health treatment within seven (7) days.

Responsibilities: Conduct outreach and provide intensive navigation services to individuals discharged from Franklin County emergency departments with mental health and/or substance use disorder diagnoses. Ensure timely linkage to qualified outpatient mental health and substance use treatment providers, with a focus on ensuring qualified service provision within seven days of ED discharge. Build supportive patient relationships through direct contact within the EDs, phone calls, virtual meetings, and in-person engagement to encourage successful connection to care. Assist with transportation or coordination of transportation to behavioral health appointments when appropriate. Assess barriers to linkage to treatment, including social determinants of health such as housing stability, transportation, food insecurity, and other factors that may impact access to care. Collaborate with hospital staff, community providers, and care teams to coordinate services and improve continuity of care. Participate in ongoing engagement and navigation activities within Franklin County emergency departments to support patients during and immediately following their ED visit. Maintain timely, accurate documentation and contribute to program reporting and quality improvement efforts. Qualifications Passion for serving individuals experiencing mental health and substance use challenges. Strong interpersonal, communication, motivational interviewing (MI) and relationship-building skills. Ability to work independently while collaborating with hospitals, community partners, and multidisciplinary teams. Experience in behavioral health, case management, care coordination, social work, peer support, or a related field is preferred. Valid Ohio driver's license and reliable transportation, with the ability to travel throughout Franklin County. As a healthcare provider, Southeast Healthcare is a non-smoking employer.
