About the role

The Hospice Aide provides skilled nursing care and monitors patient conditions according to the individualized plan of care. They collaborate with the RN Case Manager to educate patients and caregivers while maintaining accurate documentation of medication regimens.

Job Description

  • To continue to be an industry pioneer in delivering unparalleled care, we need a Hospice Certified Nurse Assistant/ Hospice Aide with commitment and compassion. Are you one of them? As a Hospice C.N.A /H.H.A., you’ll contribute to our success in the following ways: Provide skilled nursing care according to the individualized Plan of Care, physician orders, and direction of the RN. Monitor patient condition and promptly report changes, concerns, or safety issues to the RN Case Manager, physician, and care team. Collaborate with the RN Case Manager to address patient needs and recommend updates to the Plan of Care when appropriate. Educate patients and caregivers on self-care techniques to support safety, independence, and overall health. Document medication regimens and maintain accurate, up-to-date medication profiles. Help prevent avoidable hospitalizations and emergency room visits through proactive, coordinated patient care. Provide compassionate, high-quality care while maintaining positive communication with patients, families, physicians, and the care team. What is Required? High School Diploma or GED equivalent required 1 year of experience as a Hospice Aide or Nursing Assistant is required State-established or other Hospice Aide training program that meets the requirements of 42 CFR §484.36 and a competency evaluation program Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations. Experience in a home health environment is preferred What's in it for you?
