The Peer Navigator connects patients with HIV, HCV, and STI prevention and treatment services while providing recovery-focused navigation and care coordination. The Peer Navigator connects patients with HCV, HIV, and sexually transmitted infection prevention, testing, treatment, and related services, including substance use disorder treatment, medication for opioid use disorder, behavioral healthcare, insurance, and patient-assistance programs.

The role provides navigation, education, HIV/HCV testing, peer recovery support, appointment assistance, and encounter documentation in the electronic health record and TN-WITS.

This is a fully in-person position primarily based on the Syringe Services Program mobile unit, with clinic and community-outreach responsibilities.

The Peer Navigator/Healthcare Navigator provides patient education, testing support, care coordination, and recovery-focused navigation for individuals receiving services through the clinic, mobile SSP unit, and community outreach settings.

This position supports HIV, HCV, STI, and PrEP-related services; assists patients with insurance coverage, patient-assistance programs, appointments, referrals, and follow-up care; and helps link patients to infectious disease care, substance use disorder treatment, medication for opioid use disorder, mental health services, and other health and social-service supports.

The role requires accurate documentation in the electronic health record, TN-WITS, and program logs, as well as participation in reporting, meetings, supervision, training, and related program activities.

The ideal candidate has knowledge of harm reduction, recovery support, substance use disorders, and HIV/HCV/STI prevention and treatment, and is able to provide respectful, trauma-informed, nonjudgmental support while maintaining confidentiality, professional boundaries, and client self-determination.
