About the role

The Registered Nurse will provide skilled nursing care in patient homes and respond to after-hours patient needs. They are also responsible for clinical documentation, chart reviews, and supervising the care provided by nurse aides and LPNs.

SHIFT: M-F for on call visits.

No additional on call requirement SUMMARY: The Registered Nurse, On-Call will support a team of nurses and patients by responding to after-hours patient needs.

They will have the opportunity to provide one on one patient care and work at the top of their license.

They’ll be part of an interdisciplinary team that focuses on providing compassionate quality care and producing positive outcomes for our patient population.

The RN On-Call will provide skilled nursing care in a patient's home as directed by the attending physician.

If there are minimal to no after-hours needs during the shift, the RN On-Call will support chart review and patient phone calls from their home.

They are responsible for all practices and duties within the scope of practice outlined by the state.

KEY RESPONSIBILITIES Observe and monitor patient conditions and perform OASIS to develop an individualized care plan and adjust as needs change.

Evaluates the treatments and medical condition of the patient according to the plan of care.

Determines if the level of care being provided meets the patients’ needs.

Administer medication as prescribed by the physician and help decrease re-hospitalizations by prioritizing visits for high-risk patients.

Oversee and supervise total care of patient provided by nurse aides and LPNs.

Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges.

Educate the patient and family on the disease process using teach back methods to ensure patient and family understanding.

Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.

Completion of SOC/ROC/Recert and subsequent documentation within 24 hours.

Collaborate with the interdisciplinary team and physicians to ensure optimal care is provided.

Complete chart reviews and answer patient calls, to support patient care and regulatory compliance.

Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.

Attend in-service training and mandatory agency meetings.

Other duties as assigned.

Qualifications

Minimum one-year experience as an RN in an acute care setting. Home care experience preferred. Ability to effectively communicate and create positive impressions with patients, families, physicians and co-workers. Ability to remain calm, have patience and be accommodating. Compassionate and caring while working with patients. Knowledgeable on nursing best practices. Ability to make appropriate nursing judgments. Ability to identify a situation and handle it with the best possible solution. Detail-oriented and observant. Ability to work autonomously and have efficient time management. Ability to follow directions and work as a team member. Valid driver’s license and auto insurance with your name as a listed driver. Designated workspace at home to assist with chart reviews. EDUCATION AND CREDENTIALS Registered Nurse with current license in the state of employment. Requirements

Candidates must be a Registered Nurse with at least one year of experience in an acute care setting. A valid driver's license, auto insurance, and a designated home workspace for remote tasks are required.
