The Physical Therapist provides physical therapy services to patients with various diagnoses as prescribed by a licensed physician.
1. Plan of Care / Delivery of Treatment
> a. Establishes a plan of care that corresponds to the proper diagnosis and > functional limitations. > b. Delivery of treatment is effective / appropriate and corresponds to plan > of care. > c. Identifies contraindications to the plan of care and informs the manager > and physician as indicated. >
2. Documentation
> a. Documentation is completed on time > b. Documentation is legible. > c. Documentation errors are very rare. >
3. Productivity
> a. Units produced correspond to hours worked. > b. Helps other therapist complete their case load prior to leaving work. > c. Informs the manager when there are openings in the schedule. >
4. Technical Knowledge / Skills
> a. Able to perform all job duties as described in the department specific > competencies. > b. Readily identifies contraindications to plan of care. > c. Resourceful. Able to solve problems. Requests guidance from the > manager when indicated. >
5. Reliability / Dependability
> a. Nonscheduled time off is limited to sickness and emergencies > b. Scheduled work shifts are punctual > c. Adapts schedule to facility needs when indicated >
