What you'll do

As a Licensed Practical Nurse (LPN), you will

Provide comprehensive oversight, coordination, and nursing care for program participants.

Maintain nursing documentation and medication records in accordance with agency and regulatory requirements.

Administer medications and ensure medications are taken as prescribed and documented in the electronic MAR system.

Provide direct nursing services including wound care, tracheostomy care, diabetic counseling, and other health-related interventions.

Assess residents' physical health and emotional support needs and respond appropriately.

Collaborate with physicians, healthcare providers, families, and interdisciplinary team members to coordinate care.

Educate individuals on health conditions, medications, treatment plans, and self-care strategies.

Maintain confidentiality and communicate significant concerns to the Nurse Manager and Program Director.

Qualifications

Required

  • Current Pennsylvania LPN license
  • Current CPR and First Aid certification
  • Valid driver's license
  • Act 33/34 clearances
  • FBI clearance, as required
  • Completion of all trainings required by licensure
  • Ability to complete a physical exam, TB test, and Hepatitis B titer

Preferred

  • Experience working in behavioral health, residential, or community-based healthcare settings
  • Experience with electronic medical records and medication administration systems
  • Schedule
  • Full-Time
  • Schedule flexibility required
  • Evening and weekend availability may be required based on business needs
  • Work location and hours may vary to support operational needs
  • Physical requirements
  • Ability to exert up to 50 pounds of force occasionally, up to 20 pounds frequently, and up to 10 pounds constantly
  • Ability to stand, sit, walk, climb stairs, reach, kneel, stoop, crouch, crawl, push, pull, and perform repetitive motions
  • Ability to perform essential job functions with or without reasonable accommodation
