About the Role

The Pharmacist is responsible for the safe, effective, and compliant management of medication therapy within a hematology-oncology physician office and outpatient infusion center. This position provides clinical pharmacy oversight of chemotherapy, immunotherapy, targeted therapies, supportive medications, and other high-risk medications administered within the practice.

The Pharmacist works collaboratively with physicians, advanced practice providers, nurses, medical assistants, and administrative personnel to ensure that oncology treatment regimens are clinically appropriate, accurately prescribed, safely prepared, properly administered, and appropriately monitored.

The Pharmacist will provide oversight of the infusion center's medication-use processes, including regimen review, treatment plan verification, medication preparation and dispensing, inventory management, staff education, medication safety, quality assurance, and regulatory compliance.

Minimum Qualifications

  • Doctor of Pharmacy (Pharm.D.) degree from an accredited college or school of pharmacy preferred.
  • Bachelor's degree in Pharmacy from an accredited program may be considered based on applicable state requirements and organizational needs.
  • Comprehensive understanding of chemotherapy, immunotherapy, targeted therapies, biologics, and supportive-care medications.
  • Knowledge of oncology treatment protocols and evidence-based treatment standards.
  • Strong knowledge of hematology and oncology pharmacotherapy.
  • Experience with chemotherapy and hazardous drug handling required.

Preferred Qualifications

  • Minimum of 3 years of pharmacy experience.
  • Additional oncology, chemotherapy, hazardous drug, or medication safety certifications.
  • Prior experience in hematology-oncology, ambulatory oncology, outpatient infusion, hospital oncology, or specialty pharmacy.
  • Prior experience overseeing an infusion center or oncology medication program.
  • Ability to educate clinical staff and communicate complex medication information clearly.
  • Proficiency with electronic medical records and pharmacy/medication management systems.

Responsibilities

  • Review and verify chemotherapy, immunotherapy, biologic, targeted therapy, and supportive-care medication orders prior to preparation and administration.
  • Evaluate treatment regimens for appropriate drug selection, dosing, sequencing, frequency, duration, and route of administration.
  • Provide pharmacy oversight for the medication-use processes within the outpatient infusion center.
  • Participate in the development and maintenance of standardized chemotherapy and infusion protocols.
  • Assist with the development of treatment plans, order sets, and standardized medication regimens within the electronic medical record.
  • Monitor infusion center medication workflows and identify opportunities for improved efficiency, safety, and patient care.
  • Assist physicians and clinical leadership with development, review, and maintenance of standardized regimens.
  • Evaluate treatment plans for appropriate dosing, sequencing, frequency, and supportive-care requirements.
  • Provide oversight of the safe handling, preparation, storage, transportation, and disposal of hazardous medications in accordance with applicable standards and regulations.
  • Serve as a pharmacy resource for physicians, advanced practice providers, nurses, and other clinical personnel.
  • Provide education to clinical staff regarding chemotherapy, immunotherapy, targeted therapies, supportive medications, medication safety, and new treatment regimens.
  • Prepare and/or administer medications appropriate for patient treatment plan as ordered by the provider.
  • Maintain accurate patient records and documentation in accordance with legal and ethical guidelines.
  • Stay current with advancements in pharmacology, clinical pharmacy practices, and healthcare regulations.

Requirements

Candidates must hold a Doctor of Pharmacy (Pharm.D.) or Bachelor's degree in Pharmacy and possess strong knowledge of oncology and hazardous drug handling. A minimum of 3 years of pharmacy experience is preferred, along with proficiency in electronic medical records.
