Job Overview The Case Manager II is to provide comprehensive case management and outreach services to a diverse youth population.
The Case Manager follows the program’s Scope of Work activities to achieve the program goals through interviewing, counseling, assessment, caseload organization, mentoring, and management.
The Case Manager also provides work oversight and mentoring to other members of the department and/or volunteers as needed.
Minimum Requirements Bachelor’s degree in Social Services, Sociology, Human Services, Counseling, or related field required.
Minimum 3 years of experience required working with adolescents in under-served communities or public assistance programs.
Bilingual Spanish/English preferred due to the population served.
A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.
