Minimum Qualifications

  • Bachelor’s degree in Social Work, Human Services, Psychology, or a related field.
  • Minimum of two years of experience in case management or social services within a healthcare or community setting.
  • Knowledge of mental health principles, crisis intervention techniques, and child welfare systems.
  • Strong understanding of discharge planning processes and community resource networks.
  • Excellent communication, documentation, and organizational skills.

Preferred Qualifications

  • Master’s degree in Social Work (MSW) or related discipline.
  • Certification in Case Management (CCM) or equivalent credential.
  • Experience working with diverse populations, including children and families in child welfare contexts.
  • Training in conflict resolution and trauma-informed care approaches.
  • Familiarity with electronic health records (EHR) and case management software.

Responsibilities

Requirements
