Overview

The Home Care RN is responsible for performing skilled nursing visits, including SOCs, recerts, and discharges, while implementing care plans and educating patients. They must also maintain accurate EMR documentation and coordinate care with physicians and other healthcare staff to prevent avoidable hospitalizations.

The Licensed Outpatient Therapist will provide virtual therapy to individuals, families, and couples, utilizing DSM-5 criteria for assessments and developing client-centered treatment plans. Responsibilities also include delivering crisis intervention and participating in clinical supervision and team check-ins.
