Overview

The RN Care Coordinator supports patients with chronic conditions and complex needs by facilitating communication, coordinating services, addressing barriers, and providing education and guidance. Duties include documenting in the chart, engaging in critical thinking to meet patient needs, and supporting chronic disease management through patient evaluation and monitoring compliance with the plan of care.
