Overview

The Registered Nurse will plan, organize, and direct compassionate home health care while managing patient symptoms and educating families. They are responsible for performing comprehensive assessments, developing individualized care plans, and maintaining accurate clinical documentation.

Overview

The Weekend Registered Nurse assumes full nursing responsibility for delivering the Plan of Care for each home health patient, continuously evaluating performance to increase productivity and quality of care. Primary duties include making initial nursing evaluations for hospice eligibility, identifying and re-assessing patient needs, initiating and coordinating the plan of care, and documenting patient responses.
