Overview

The Physical Therapist will plan and administer prescribed physical therapy treatments in patients' homes to maximize physical function, which includes assessing strengths to guide treatment goals and developing rehabilitation programs consistent with the physician's Plan of Treatment. This role also involves providing clear care instructions to PTAs and Home Health Aides, overseeing their work, and thoroughly documenting and reporting patient progress to all relevant parties.

Overview

Provide occupational therapy services to patients under the supervision of a licensed Occupational Therapist to improve functional independence. Manage patient cases, maintain clinical documentation, and ensure compliance with state and federal regulations.
