Overview

The Physical Therapist Assistant provides direct physical therapy services in patient homes according to the established Plan of Care. They are responsible for instructing patients and caregivers on safe device usage and reporting any changes in patient status to the supervising Physical Therapist.

Overview

The Licensed Practical Nurse verifies the Plan of Care before each visit and provides care based on RN delegation, physician orders, and established guidelines, incorporating patient care goals into nursing visits through documentation. They must communicate significant findings, problems, and changes in condition to the RN Case Manager, physician, or supervisor, and implement patient safety plans.
