The Primary Care Physician independently provides care for older adult patients with acute and chronic illnesses, taking full accountability for patient care and outcomes while leading their immediate care team. Responsibilities also include engaging with hospitalists, managing complex health needs through assessment and documentation, and actively participating in center management and provider recruiting efforts.
